WAIVER INVOLVEMENT CONSENT | I hereby consent for my child to participate in all activities they may choose whilst part of Horizon Church Youth Programs including Force, Chosen and any associated events ("the Programs"). This includes games, jumping castles, swimming, skateboarding and any other events that may be a part of the Programs. I understand that whilst every reasonable precaution will be taken to ensure the safety of my child including providing the necessary supervision, I hereby release and hold harmless Horizon Church and its incorporated entities, its staff and volunteers from any and all liability in the event of an accident, misfortune, injury, damage or loss that may occur to my child and their property whilst attending the Programs or where such damage, loss or harm has occurred as a result of the events mentioned below. TRANSPORTATION | I hereby consent for Horizon Church volunteers and/or staff to provide transport when required for my child to and from the Programs. This includes transportation with a licenced driver (provisional or unrestricted), public transport and/or any ride sharing services. MEDICAL TREATMENT CONSENT | I hereby consent for the Horizon Youth First Aid Officer to ensure proper treatment for my child. In the case of an emergency, I give permission for Horizon Church staff and volunteers to obtain medical, hospital or ambulance assistance. I understand that every effort will be made for the listed emergency contact to be notified before instituting such procedures. I agree to pay for any medical, hospital and ambulance expenses incurred on behalf of my child. PRIVACY DECLARATION | I understand Horizon Church may collect and use information about my child for the purpose of providing promotional material. I consent to my child's personal details being used for the promotion of future Horizon Church events and products. This information will be held in accordance with their privacy policy and remain confidential at all times. MEDIA WAIVER | The Programs experience will be captured in photographs, video and audio. Horizon Church reserves the right to use the material for promotional purposes. I hereby give consent for my child to be captured on photographic, video and audio media and that such content may be used on social media and any other online platforms used by Horizon Church. The unenforceability or invalidity of any clause in this Waiver shall not have an impact on the enforceability or validity of any other clause. Any unenforceable or invalid clause will be regarded as removed from this Waiver to the extent of its unenforceability and invalidity.
